RN Case Manager

StartDate: 1/12/2026 Pay Rate: $1800.00 - $2700.00

POSITION SUMMARY – RN Case Manager

POSITION DUTIES – Ensures medical necessity of hospitalization and treatment in accordance with hospital and third party payor guidelines and criteria. Ensures appropriate utilization of hospital resources. Works with other physicians and health care team members to manage care in a cost-effective and efficient manner. Responsible for the integration, coordination, and advocacy for patients and families requiring post-hospital services. Achieves planned care outcomes by planning and arranging services on discharge. Acts as liaison with Medical Staff, other departments, patient and family members and community resources to facilitate the utilization management review/discharge planning process.

MINIMUM REQUIRED QUALIFICATIONS – MA RN license; BLS; 2 years strong and recent Inpatient Hospital Case Management experience, plus experience in discharge; Must be familiar with Medicare, Medicaid, manager care and other 3rd party payor review requirements

Preferred Qualifications – Bsn

LENGTH OF ASSIGNMENT – 13 weeks

SHIFT / HOURS PER WEEK – 40 hours a week, M-F

SYSTEMS – Epic

START DATE – ASAP
